The Feedback module allows you to create a questionaire. Unlike the inbuilt Moodle Questionaire, Feedback allows you to set your own questions. This is good as the inbuilt questionaires have never really suited our needs because they have very specific uses.
The Update this Feedback page, which is the first page you edit when adding a Feedback activity to a Moodle Course, is organised cleanly and the settings are consise and easy to understand. Defaults have also been set such that for normal use you really won't have to change very much. This is great for the average teacher or instructor using the module.
The interface for adding and editing questions is intuitive and efficient. The only problem I had was when entering multiple choice rated questions. It took me a while to realise the format required (there was no help or messages provided). In hindsight it makes sense but at first it was a little confusing.
The template mechanism is useful, especially if you have a common set of questions you would like to ask regularly.
One thing I did not like was that it was hard to experiment with. I set myself as both a teacher and student in the course to play with it yet it would not let me enter any feedback as long as I was a teacher, even though I was also a student.
Conclusion: Overall a useful module.
